Contract for Canine Cryogenic Services

CONTRACT FOR CANINE CRYOGENIC SERVICES

This is a contract between Sondel Family Veterinary Clinic (SFVC) and above stated client. The purpose of this contract is to establish responsibilities, liabilities, and procedures regarding the collection and freezing of canine semen.

• Client shall provide current billing information to Sondel Family Veterinary Clinic to prevent billing complications
• All charges accrued for the cryogenic services provided by Sondel Family Veterinary Clinic shall be paid by the owner of the stud dog at time of service
• Client shall contact Sondel Family Veterinary Clinic at least 24 hours in advance before the semen is going to be used or shipped
• Client shall provide unique information to prove ownership of semen prior to Sondel Family Veterinary Clinic releasing the semen
• Client is responsible for obtaining the required shipping information, especially when shipping internationally. This includes but is not limited to: clinic address, clinic name, clinic phone number, credit card number, any required health checks/vaccinations, customs/importing/exporting paperwork and signatures.
• Client will receive no refund of the yearly storage fees if it is decided that the semen is no longer needed to be stored at any time throughout the year.

• Clinic will determine the amount of breeding doses/straws after analyzing the sample
• Clinic will store semen in a room separately locked from the rest of the clinic
• Clinic will not release the semen unless provided unique information identifying the ownership of the semen
• Clinic will send an annual bill for the storage of the semen in the beginning of the month that the collection/freeze was performed
• Clinic shall send in AKC registration once the freeze is deemed a success and owner has provided all necessary stud dog information

• Sondel Family Veterinary Clinic is not liable for unsuccessful breedings from the use of the semen frozen at the clinic. Sondel Family Veterinary Clinic recommends the insemination be performed by a trained veterinarian and staff.
• Sondel Family Veterinary Clinic is not liable for the accidental thawing of the frozen semen. This includes thawing during transport, storage tank failure, and any event beyond the control of Sondel Family Veterinary Clinic.
• Owner is liable for paying the bill for cryogenic services including annual storage fees. Any bill not paid within 90 days will result in forfeiture of the frozen semen, and will result in disposal of stored semen.
• By signing this contract, the client agrees to the above details of the contract, and is thus bound to the stipulations found in the contract.

Please provide either an e-mail address or secret answer/password (for example, mother’s maiden name, name of your first pet, etc.) that will be given to us when semen is to be released for shipping/breeding. This is for security purposes only, with the intent to eliminate the misuse of the semen stored here belonging to you. This will be asked to be provided before Sondel Family Veterinary Clinic uses/ships the stored semen.
