International Travel Consent Form

I, having elected to travel internationally with my pet, have chosen Sondel Family Veterinary Clinic (SFVC) to assist my pet and me with the required examination(s), procedures, and paperwork associated with travel. I understand that travel with pets requires specific and sometimes extremely strict guidelines and protocols from the country of destination. Because these guidelines and protocols vary and are ever-changing, I understand that it is my responsibility to research what is required from the destination I am traveling to. I agree to inform SFVC of anything required of them, give them ample time to complete the necessary paperwork, and will review all paperwork before traveling with my pet to ensure it is accurate and complete. I understand that while SFVC will make every effort to ensure that all requirements are met, I am solely responsible for all information that is submitted for travel with my pet. I will not hold SFVC or any of its team members responsible if there is any incorrect or missing information that prevents me from being able to travel with my pet.
